The shield door has been opened and fields measured at 77 uSv/h on contact at the AL. panel.

Entry  Thursday, October 12, 2017, 08:48, Travis Cave, Spent Target Vault, Standard Operation, , , Field measurements of the old source trays

Field measurements were taken of the old source trays. The old TM#2 source tray had a minimum field value of 0.50 mSv/hr and a maximum of 1.18 mSv/hr through 360 deg of rotation. The box was rotated around three times and put back in the vault. TM#1 old source tray was also removed and measured as well the field values for it were a minimum of 0.52 mSv/hr and a maximum of 1.15 mSv/hr through 360 deg of rotation. The box was rotated around three times and placed back in the vault. In both cases the box was roughly one metre from the radiation monitor.

Entry  Tuesday, January 16, 2018, 06:50, David Wang, ITE/ITW Cooling, Standard Operation, , , Field measurement on old resin tank in silo

The old resin tank has been stored in silo since last year March.  Without crane the tank up, We use the pole monitor measured the field around the bottom of the tank in silo. The average contact field on the bottom is about 4.0msv/hr. @0.5meter , the field is about  900usv/hr. It is still too hot to remove it.

Fiberglass, capton tape and tie straps have been removed from TM3.

Entry  Thursday, October 31, 2019, 10:17, David Wang, Assembly/Bench Test, Maintenance, TM2, , Febiad target gas line leak check.

Feibiad gas line for ITE and its adapter fittings for TM2 are tested today by installing them on a dummy water stem. No leak on VCO fitting, adapter o-rings, and all Swagelock fittings of this assembly. Manual valve on gas line works fine.  The positional check on gas line between TM2 and ITW will be fulfilled next Tuesday during module disconnection.

Entry  Monday, November 18, 2013, 11:54, Bevan Moss, Conditioning Station, Repair, TM2, , Fault on TCS Chill

This morning when I went to start HV conditioning CP2 and CP3 both had faults and the temperature of the water was ~27C. I reset the compressors by flipping the system reset switch. This cleared the fault and allowed the compressors to come one. To the best of my knowledge these faults happen as a result of compressor fluid pressure too low or high and temperature too low or high and lack of flow. The system cannot determine which one it was that caused the trip. The system restarted and faulted again. As I was watching this time it can be determined that the fault was the result of an over temperature event. This was the caused by the cooling fans not coming on. Which was traced to the fan VFD controllers which both read OP3 which is an Over voltage during constant speed operation. The operating manual lists these as the possible causes

The input voltage is fluctuating abnormally.

• Power network is greater than 200 kVA

• Power factor capacitor switching

• SCR switching on power network

• The drive controller is regenerating— the load causes the motor to run at a frequency higher than drive controller output frequency.

• Intermittent output phase fault

The proposed solutions are

• Install a line reactor.

• Check the input and output circuits for phase failure and rectify.

I will continue to monitor over the next few days to see if this continues to happen. If it does I will contact ESC.

Found both VFDs with OF3 fault (over voltage during constant speed operation).

Thursday, January 08, 2015, 16:37: Cycled power on the chiller to clear faults.

Entry  Wednesday, March 13, 2013, 14:30, Bevan Moss, Conditioning Station, Development, , , Faraday Cup

Dave Morris and I investigated the Faraday Cup today. It was found to be plumed in the wrong direction (fails in). This is not an issue for the conditioning station as there is nothing down stream from the cup. Dave will change the controls to reflect this.

Entry  Tuesday, July 18, 2017, 15:04, Travis Cave, Crane, Maintenance, , , Failure of the ISAC TH Crane Rotate.

Earlier in the day it was noted that the rotate function of the crane had creased operation. After David has finished the block moves. Don and I checked the Crane's control unit in the ISAC crane control room and found the breaker tripped for the rotation. The breaker was reset and the crane's rotation is now operational. If this occurs in the future only Travis Cave, Don Jackson or a TRIUMF Electrician are allowed in that control box as it is a 480Volt system. Don or I because we know what we are looking for and the Electrician because they are allowed to touch with and deal 480 V systems.

Entry  Thursday, September 12, 2024, 14:12, Adam Newsome, Waste Package/Ship, Development, , SiC#44, Failed waste emplacement at CNL: Flask #22, Pail 275 (SiC#44) Report_on_Inspection_of_Pail_275_on_2024-10-03.pdf

On 2024-09-12, CNL reported difficulty with lowering Pail 275 from F-308 flask #22 into their tile hole. The four other pails associated with this shipment were successfully emplaced. It is suspected that the issue was related to a kink in the lowering cable, causing a jam during lowering. This was observed approximately 1 year ago.

Discussions with CNL to learn more about the root case are ongoing, and this e-log will be updated when more information is learned. It is expected that this flask will be shipped back to TRIUMF and the pail will need to be repacked.

Update 2024-09-12: email from A. Swan at CNL: "4 of the 5 flask were emplaced successfully with F308 #22 SiC#44 having an unsuccessful pull test and left within the flask."
This is the extent of the information they have provided and is not sufficiently helpful to determine what the issue is.

Updated 2024-10-03: an investigation was performed (work permit C2024-09-26-10). It was determined that the root cause of the issue is attributed to an assembly error - the cable was incorrectly routed through the holder part. See attached report for full details.

Entry  Monday, July 07, 2014, 14:45, Grant Minor, ITE, Maintenance, TM3, SiC #30, FEBIAD gas line check ITE_Febiad_Gas_Line_Check_7July2014_arrows.jpg

There were some problems this morning with provision of FEBIAD gas to the the target at ITE.  It was suspected that the gas line was improperly connected to the module, or that the valves at the module were not opened.

Remote Handling was asked to open the HV cover on TM3 at ITE and check the connection of the FEBIAD gas line.  Grant Minor, Maico Dalla Valle, Dan McDonald, and Anders Mjos entered the Target Hall before lunch on Work Permit I2014-07-07-1.

Maico entered ITE with suit and respirator, disengaged / disconnected the interlocks and removed the HV cover.  He inspected the FEBIAD gas line and found that it was properly connected, and that the valves were both open.  The gas line is shown in the attached image, with both valves marked in red.

Grant and Anders went into the electrical room to examine the supply gas bottle and supply circuit, and found that the supply gas pressure to the regulating valve was low.  It was concluded that the problem was not at the Target Module / Target Station.

Grant returned to the Target Station and instructed Maico to replace the HV cover.  After this was done, Grant confirmed with Jon Aoki over the phone that the interlock switch signals were both reading "OK".

Friedhelm Ames made an entry in today's ISAC Ops e-log regarding the supply pressure to the regulating valve.
