Upon attempting to lower Pail 211 into their tile hole, CNL encountered issues with the pail getting stuck. They shipped the pail back to TRIUMF and requested we perform an investigation. The investigation report is attached.

The pail contents must be re-packaged into a new pail.

Note: the F-308 is currently sitting in the Meson Hall warm cell at approximately 250 uSv/h at 0.5 m.

Dose for primary worker (Chad Fisher) who inspected the cable, unbolted the flask lid, and handled the pail briefly: approx. 104 uSv

Dose for secondary worker (Danka Krsmanovic) who swiped the pail and flask to check for contamination: approx. 70 uSv

Tried to leak check TM4 Ta57# at TCS. LD failed to pump down properly at fine range. It always tripped at 4 torr. I cheeked all vacuum lines seals and adapters on top of module to LD. Everything is good. Chad said the VCRs at SHC can not be tightened any more. Discussed with Anders, we decide to move module TCS and helium pressure test there.  The same difficult happened last time on TM4 UCx24# at SHC when leak check. But,it is solved by pressurize the line at SHC last time.

The HS line was pressurized with 70 psi helium on Oct 04th at TCS.. LD 0,00X E-9atm.cc/sec no change in 2 minutes. No leak.

Ta #47 has been removed from TM4.

One containment box cover bolt would not come out. Upon attempted removal with the stud remover the bolt sheared...which is not uncommon...just hasn't happened in many years...CBB being stuck or shearing.

Nine pin connector shows the typical "arcing" mark.

TM4 with Ta#42 was HV conditioned to 35kV with the optics tray at 17.5kV. There was some vacuum response while ramping but well within normal. Attached is a screen shot and the associated data.

Entry  Monday, May 27, 2013, 10:35, Bevan Moss, Conditioning Station, Standard Operation, TM4, Ta #40, Pumps Started HV testing to follow

The pumps have been turned on at ~9:30 this morning. Current values are 6.61 Mohm TCS:DIC1 @19.4 C. HV testing will start momentarily.

The HV conditioning reached a maximum of 44kV today before a spark took out the canbus card. I will attach the data once I have had a chance to format it.

Roughing started at 12:06. It was believed there was a problem with the compressed air but there was none, TM1 doesn't leak and so you cannot hear that the connection is made.

Roughing stopped as the module and cover requires modifications that require the vacuum to be opened.

TM1 started venting at 4:15 pm with Argon. I will leave the valve open over night.

TM1 with Ta #37 target was moved from ITW to the south hot cell. The rotation of TM1 may have been off slightly during the final removal from ITW but it was minimal. Entry into the south hot cell went with out incident. This was my first time moving the module without the presence of Travis Cave.

Entry  Thursday, May 15, 2014, 14:38, David Wang, ITE, Repair, TM3, TM3, Bias short on ITE

Anders and I opened the HV cover in ITE and did  a visual check inside ITE HV  cover and top of TM3.Bill was with us and took the pictures. There was nothing obviously wrong when we opened the HV cover in ITE. We disconnected TM3 high current cables from ITE. Then we did an electrical check on TM3.The TM3 was good and passed megga test (465Mohm at 500V). The module was good and was separated electrically from the ITE at that moment. Then, we went back to electrical room and measured the resistance between bias and ground with ground bar opened and power supply disconnect. It was still shorted at that moment .We thought the short is located inside HV chase between electrical room and ITE. Then, we went back to ITE again and reconnected TM3 back to ITE. We wiped two blue nylon insulators between bias and ground, and no obviously spark track or dirty on them. We put HV cover back in ITE locked area, and went back to electrical room again. We decided to do a final electrical check there. We thought the short should be still there and we just want to confirm it . But this time the short  is gone. Everything goes back to normal.   The short disappeared and We test bias to 5KV and the current looks normal now.

TM4 Ta#40 HS leak check is done at SHC . No leak has been found so far. There will be  a helium pressure test on it at CS.
