Thursday, September 12, 2024, 14:12, Adam Newsome, Waste Package/Ship, Development, , SiC#44, Failed waste emplacement at CNL: Flask #22, Pail 275 (SiC#44)
|
On 2024-09-12, CNL reported difficulty with lowering Pail 275 from F-308 flask #22 into their tile hole. The four other pails associated with this shipment were successfully emplaced. It is suspected that the issue was related to a kink in the lowering cable, causing a jam during lowering. This was observed approximately 1 year ago.
Discussions with CNL to learn more about the root case are ongoing, and this e-log will be updated when more information is learned. It is expected that this flask will be shipped back to TRIUMF and the pail will need to be repacked.
Update 2024-09-12: email from A. Swan at CNL: "4 of the 5 flask were emplaced successfully with F308 #22 SiC#44 having an unsuccessful pull test and left within the flask."
This is the extent of the information they have provided and is not sufficiently helpful to determine what the issue is.
Updated 2024-10-03: an investigation was performed (work permit C2024-09-26-10). It was determined that the root cause of the issue is attributed to an assembly error - the cable was incorrectly routed through the holder part. See attached report for full details.

Monday, July 07, 2014, 14:45, Grant Minor, ITE, Maintenance, TM3, SiC #30, FEBIAD gas line check
|
There were some problems this morning with provision of FEBIAD gas to the the target at ITE. It was suspected that the gas line was improperly connected to the module, or that the valves at the module were not opened.
Remote Handling was asked to open the HV cover on TM3 at ITE and check the connection of the FEBIAD gas line. Grant Minor, Maico Dalla Valle, Dan McDonald, and Anders Mjos entered the Target Hall before lunch on Work Permit I2014-07-07-1.
Maico entered ITE with suit and respirator, disengaged / disconnected the interlocks and removed the HV cover. He inspected the FEBIAD gas line and found that it was properly connected, and that the valves were both open. The gas line is shown in the attached image, with both valves marked in red.
Grant and Anders went into the electrical room to examine the supply gas bottle and supply circuit, and found that the supply gas pressure to the regulating valve was low. It was concluded that the problem was not at the Target Module / Target Station.
Grant returned to the Target Station and instructed Maico to replace the HV cover. After this was done, Grant confirmed with Jon Aoki over the phone that the interlock switch signals were both reading "OK".
Friedhelm Ames made an entry in today's ISAC Ops e-log regarding the supply pressure to the regulating valve.

The five spent target F308 flasks where gone through in accordance with Nordion guidelines, the sixth spare is remains in the Hot Cell area. All flasks where in reasonable shape with two needing weld repairs on the pallets. I replaced all the 3/4" pallet to flask fasteners with new Grade 8 bolts nut and washer combo. Flasks have been moved into the Hot Cell area awaiting movement into the Target Hall. |

Six F-308's where received from Nordion #6,#15,#13,#16 all contained contamination. Loose contamination was removed with some fixed contamination remaining levels are low enough to allow the transport of spent targets. flasks where serviced as per Nordion Document IS/PP 0009 F308, IS/PP 0981 F308. Flasks are now located in the north hot cell area. |

5 pails were moved from the mini-storage vault into F-308's
At one point during the loading the mini-storage vault door would not close. Cause was a blown fuse, which was replaced and operations were able to continue as usual. Possible reason the fuse blow was a tilted pail that may have become jammed between the tray and the roof of the mini-storage vault.
[Edit 2022-12-01, Adam Newsome]: the pails which were removed were: 220, 227, 233, 235, 238 |

The F-308 have been cleaned and serviced. There were three F-308's that came back with some contamination.#13, #15 and #19 were found to have contamination. They were decontaminated. All of the loose contamination was removed. There is still some fixed contamination. The levels of contamination are low enough to still be used for shipping spent targets. They have been moved to the north hot cell area and are ready for to be filled with spent targets. |

Wednesday, October 21, 2015, 11:58, Anders Mjos, South Hot-Cell, Development, TM2, ZrC#7, Extraction Electrode inspection
|
The failed extraction electrode was inspected in the hot cell this morning by Anders and Chad. The following electrical measurements were taken
- EE on target assembly as recieved from TM after online operation. 14 Ohm
- EE removed from target assembly and placed on conductive flat surface. ~1 G-Ohm at 1kV
- EE installed back onto target assembly (fasteners not tightened).~ 0.8 G-Ohm at 1 kV
There was some material deposited on the EE. Some of which has fallen off - possibly during removal of the electrode from the target assembly. It seems the cause of the short was material build up between plasma electrode and extraction electrode. See photos.
The heat shield lid was removed after lunch. This revealed a broken target. The target may have broken after operation (in hot cell).
